New Nicotine Products Worry Health Professionals in Spain

Health experts highlight the growing concern over alternative nicotine delivery systems like vapes and oral nicotine pouches, which are increasingly popular among younger demographics and pose significant health risks.

Health professionals in Spain are voicing growing concerns over the evolving landscape of nicotine consumption, as alternative products like vapes, smokeless tobacco, water pipes, and oral nicotine pouches gain traction. These newer forms of nicotine delivery are often perceived by the public as less harmful than traditional cigarettes, a misconception that health experts are actively working to counter.

New Forms, Old Dangers

Guadalupe González Valladares, a pulmonologist at the Pontevedra and O Salnés health area and head of the smoking cessation clinic at Montecelo Hospital, highlighted this issue during an institutional event for World No Tobacco Day. She emphasized that while traditional cigarettes are widely understood to be harmful due to decades of public health campaigns and legislation, the risks associated with newer nicotine products are not as widely recognized.

«With traditional cigarettes, campaigns and laws have made society aware that they are harmful,» explained González Valladares. «But the same is not happening with new forms of nicotine consumption. Products are appearing that are wrongly perceived as less harmful.»

Among these, she specifically mentioned electronic cigarettes (vapeadores), smokeless tobacco, water pipes, and nicotine pouches. These small pouches, designed for oral absorption, are particularly concerning due to their discreet nature and appeal to younger users. «A teenager can have a nicotine pouch in class without it being hardly noticed,» she warned.

Industry Strategy and Youth Appeal

González Valladares attributes the diversification of nicotine products to the tobacco industry’s strategy in response to declining traditional cigarette consumption. «The consumption of traditional cigarettes is decreasing, and companies have diversified their products, primarily targeting the young population as new consumers,» she stated. The appeal of these products is further enhanced by their variety of flavors and scents, making them particularly attractive to adolescents.

Age and Health Impacts

Age plays a significant role in the patterns of nicotine consumption and its health consequences. According to González Valladares, the highest rates of consumption for both traditional cigarettes and, notably, electronic cigarettes are observed up to the age of 45. Beyond this age, individuals tend to become more aware of their health and begin to experience clinical problems.

«The first ailments start to appear,» she summarized, listing conditions such as Chronic Obstructive Pulmonary Disease (COPD), asthma exacerbations, early lung cancer diagnoses, ischemic heart disease, heart attacks, angina, strokes, arterial circulation problems, hypertension, and other risk factors like high cholesterol.

Beyond Smoke: Risks of Smokeless Products

It is crucial to understand that the dangers of nicotine consumption are not limited to inhaled smoke. Products that do not involve combustion can also lead to dependence and serious illnesses. «Dependence is generated by nicotine,» González Valladares reminded. For oral absorption products like nicotine pouches or chewing tobacco, she warned of their association with oral cavity diseases, including oral cancer.

Vapeadores, despite often being marketed as a safer alternative, are also not harmless. They can contain heavy metals such as lead, nickel, and tin, as well as substances like acetaldehyde and formaldehyde, in addition to numerous additives. «It is important to convey that these are not harmless products, that they can cause pathologies, and that they cause addiction,» she insisted.

Comprehensive Approach to Smoking Cessation

The Pontevedra and O Salnés health area is addressing the issue of tobacco use through its smoking cessation clinic and a comprehensive plan. This plan includes attention to the general population, hospitalized patients, and healthcare workers across three hospitals. The ultimate goal, as outlined by the pulmonologist, is to prevent, treat, and reduce the prevalence of smoking and nicotine addiction.
